Why I Built CascadEffects and Why It's Working
Add a short descrI spent years as a consultant, and I was proud of my retention. Clients wouldn’t leave. I wore that like a badge of honor. Then I stepped into something different, launching MyOrthoVA and leading the global healthcare team at MEDVA, where we supported roughly 800 customers with virtual assistants from the Philippines and Latin America. I got to see healthcare practices at scale (ortho and dental included), from the inside, across hundreds of different operational models. And what I kept seeing was the same thing: doctors and practice owners throwing staffing at a problem that staffing was never going to solve. A new hire doesn’t fix a broken system. A virtual assistant doesn’t replace a missing leader. And yet practice after practice was reaching for the same lever, more people, hoping it would be the answer. I missed working closely with practices. But when I thought about going back to consulting, something nagged at me. That retention rate I’d been so proud of? I started to see it differently.iption.

If I was really a great consultant, wouldn’t the practice eventually not need a consultant anymore? That question changed everything.
The Problem with the Consulting Model
Here’s the uncomfortable truth about traditional consulting in orthodontics: high retention often isn’t a sign of quality. It’s a sign of dependency. I watched it play out over and over. A consultant comes in, identifies the problems, builds a plan, and starts driving results. The practice improves. Everyone’s happy. The engagement continues. But pull the consultant out and what happens?
The systems atrophy. The team reverts. The progress stalls. Not because the consulting was bad but because the practice never built the internal leadership to sustain it. The problem isn’t the advice. It’s the model. Traditional consulting tells you what to do, then leaves you to figure out how. Or worse, it stays forever, becoming a crutch the practice can’t walk without. I knew I needed to build something fundamentally different.

Doctor-Dependency Is the Biggest Growth Ceiling
The practices struggling to scale share one common denominator: everything runs through the doctor. Any disruption, staffing, health, growth plans, becomes a crisis. Building leadership depth is the most urgent operational priority in 2026.

Tech Drag Is an Operations Problem
Practices are blaming software for chaos that belongs to undefined workflows. The tool isn't the problem, the absence of ownership, documentation, and standards is. Adding technology without operational structure amplifies the breakdown, it doesn't fix it.

Starts Are Being Lost at the Front Desk
Case acceptance gaps are rarely exclusively in the TC room. By the time treatment is presented, the patient's trust decision is mostly made. The intake experience, call handling, response time, lobby culture, is where the case is won or lost.

There's a pattern across the practices that are growing right now, and it's not what most people expect. It's not the marketing. It's not the tech stack. It's not even the team. It's financial clarity. The doctors who are winning can tell you their revenue per new patient start, their overhead percentage, their exam-to-contract conversion rate, and what it costs to acquire a patient. Not approximately. They know.
For vendors and partners, this matters too. The practices most likely to invest in your product are the ones with operational clarity, they know what a dollar spent should return. Reactive buyers churn. Informed buyers grow. This year, financial intelligence is the competitive advantage. Practices that have it will grow with intention. The rest will stay busy without building anything.
