Frequently asked questions
What exactly does CascadEffects do?
CascadEffects operates as an embedded operational partner for orthodontic and dental practices, working alongside doctors and leadership teams to build the systems, accountability structures, and team capacity that let a practice grow without depending entirely on the owner. Rather than delivering a one time recommendation and stepping away, our team works inside the practice, often shoulder to shoulder with the leadership team, to design infrastructure that fits how that specific practice actually operates. Our work spans three connected lanes: diagnostic analysis, hands on fractional COO partnership, and virtual assistant placement, each flowing from the last. Most practices enter through whichever lane matches their most pressing need.
CascadEffects also provides dedicated virtual assistant placement, either as a standalone service or alongside a fractional COO engagement. Every VA is recruited specifically for the role a client needs, vetted for skill alignment and cultural fit, and supported with structured onboarding and ongoing coaching to keep performance on track. Our placements span industries and skill sets, from front desk coordinator and billing support to paid ads manager, development project manager, and beyond. If a transition ever becomes necessary, our Smooth Replacement Program keeps operations moving without a gap.
How is CascadEffects different from a traditional consultant or coach?
Consultants typically deliver recommendations and hand you a report to implement on your own. Coaches focus mostly on the doctor's mindset and personal development. A fractional COO does neither in isolation. At CascadEffects, we step inside the operation as a working executive, owning outcomes alongside the practice rather than advising from the sidelines. That means building the systems, training the team, sitting in on the hard conversations, holding leaders accountable, and staying long enough for the changes to actually stick. The distinction matters because most operational problems in a practice are an execution gap, not a knowledge gap, and a binder of recommendations rarely survives a busy workday.
Do you work with just dental practices, just orthodontic practices, or both?
Orthodontics is our core specialty, reflected in our founder's background and ongoing fellowship work, but the same operational principles translate directly to other healthcare practices and specialty groups, where the underlying challenges, such as team accountability, scheduling efficiency, and patient experience consistency, look similar. CascadEffects works with single location practices, multi location groups, and various specialties, tailoring the operational build to the specialty and growth stage of the practice rather than applying a single template. If you are unsure whether your practice is a fit, that is exactly the kind of question an initial conversation with us is built to answer.
Do you work with solo doctor practices or only multi location groups?
Both. Solo doctor owners and multi location groups face different operational challenges, but the underlying work is the same: build systems and leadership so growth does not depend on the doctor being in the building. Engagement scope is scaled to the size and complexity of the practice rather than applied as a single template regardless of structure. A single location practice and a five location group both need clear roles, accountability, and consistent systems; they simply need different amounts of structure to get there.
Do you work with practices in person, remotely, or a combination of both?
Most of the work is delivered remotely, and that's by design. When a consultant walks into a practice to observe, it's like the principal walking into a classroom. Everyone is on their best behavior, the systems seem to run smoothly, and the consultant leaves with an impression that doesn't reflect what a normal day actually looks like. Remote engagement removes that dynamic. It holds the leadership team accountable to identify issues themselves, follow through on adjustments, and build the skills that sustain improvement long after the engagement ends. The result is leadership development that's real, not performative.
Do we need to be a certain size or revenue level to work with CascadEffects?
There is no fixed revenue threshold. What matters more is whether a practice has reached a point where growth is creating operational strain, where the owner is carrying more of the day to day than they should be, or where leadership exists but is not yet functioning as a true accountability structure. Some engagements start with a single location practice preparing for its first major growth push. Others begin with a multi location group whose systems have not kept pace with its footprint. The Practice Performance Analysis is often the clearest way to establish where a specific practice stands before committing to deeper work.
How does pricing work?
Do you charge a flat fee or a retainer? Engagement structure depends on the scope of work rather than a single published rate card, since a diagnostic analysis, a six month operational build, and an ongoing advisory partnership all require different levels of involvement and time. Rather than publish generic pricing that would not reflect the realities of a given practice, our CascadEffects team reviews specifics such as practice size, number of locations, current team structure, and the depth of work required during an initial conversation, then proposes an engagement scope and investment that matches the actual work involved. This keeps pricing tied to value delivered rather than a one size fits all package.
What does the engagement process typically look like from first contact to results?
Most relationships begin with a discovery conversation to understand where the practice stands and what is creating friction. From there, many practices choose to start with a Practice Performance Analysis or Local Competitive Analysis to establish a clear baseline before committing to deeper work. Practices ready for hands on transformation move into the Six Months of Deep Work engagement, where CascadEffects operates as an embedded fractional COO. Some practices bring on Virtual Assistant support in parallel to extend team capacity. The sequence is not rigid: some practices start directly with fractional COO work, but the through line is always starting with clarity before building structure.
Is the initial consultation actually free, and what happens during it?
Yes, the initial consultation is complimentary and functions as a discovery conversation rather than a sales pitch. The purpose is to understand the specific pressures a practice is facing, whether that is operational strain from growth, team accountability gaps, unclear market positioning, or capacity constraints, and to identify which starting point makes the most sense. Not every practice that reaches out is a fit for every service, and part of the value of this conversation is an honest assessment of what would actually move the needle rather than defaulting to the most expensive option available. If the fit is not right, that gets said directly rather than stretched into a longer sales process.
